Enrollment Authorization
You understand that enrollment in the Delta Dental and Vision Plan is voluntary. By enrolling, you authorize Aegis Administrative Services, Inc. to charge your credit card for the premium associated with the coverage and payment option you selected.
The premium will be charged on a recurring basis and will remain in effect until you cancel your coverage in writing.
By enrolling, you agree not to dispute a recurring billing charge with your bank as long as the transaction matches the terms provided during the enrollment process. If you dispute a charge, you agree to be responsible for the $30.00 dispute fee assessed by the merchant.
Effective Date of Coverage
Your coverage will become effective on the first day of the month following your enrollment date.
Cancellation Policy
Your cancellation will become effective at the end of the month in which the cancellation request is made.
Retroactive terminations are not permitted.
